30 Years of Health Leads

Celebrating the people and partnerships building health, well-being, and dignity for every person, in every community.

Three women

Thirty years ago, Health Leads began with a simple but radical idea: health is shaped by far more than what happens inside a doctor’s office.

Since then, we’ve worked alongside communities, health systems, caregivers, public health leaders, social service partners, funders, volunteers, and advocates to remove barriers to the resources everyone needs to be healthy.

This year, we’re celebrating the people who made that work possible — and committing to the next 30 years of community-led change.

Join us as we honor where we’ve been, celebrate how far we’ve come, and look ahead to what’s next.

Our Journey: 30 Years of Community-Led Health

From early work connecting families to essential resources, to today’s community-led and co-designed initiatives around the most pressing equity challenges, Health Leads has continued to evolve with the communities and partners we serve. Scroll through our journey to this moment and meet our supporters along the way.

Through it all, one belief has guided us: communities are healthier when the people closest to the challenges have the power and resources to design the solutions.

Michael Tang, MD, MBA

Chief Medical Officer, Blackbird Health · Health Leads Alum & Board Member

Michael has seen Health Leads from nearly every angle — student volunteer, staff leader, community-based physician, and now newly appointed Board Member. He is Chief Medical Officer at Blackbird Health.

On his 30-year journey with Health Leads

“I literally grew up with Health Leads. As a volunteer, I saw the importance of bringing together communities, providers and supporters to deliver key services for children. As a staff member, I worked to build the organization.

As a community-based physician, I saw Health Leads’ unique position to bring new workforces into addressing health inequities — from volunteers (like me) to community health workers — and disseminate best practices across the country.”

How Health Leads is meeting this moment

“Our country is in a unique time, and I am excited that Health Leads’ work continues to meet the challenges of today — drawing on the lessons learned over the past 30 years to address the urgent health and social inequities we face.”

Barry Solomon, MD, MPH

Johns Hopkins University School of Medicine

In 2006, Dr. Solomon helped open a single Health Leads desk in the Harriet Lane Clinic in Baltimore. Twenty years later that work lives on as Hopkins Community Connection — one of our longest-running partnerships, still innovating to meet the essential needs of children and families.

On the journey from a single desk to a new standard of care

“In 2006, we began our Health Leads journey to support families with unmet social needs in the Harriet Lane Clinic. What began with undergraduate student volunteers grew into a professional team — community health workers, case managers, and site coordinators — in pediatric and adult practices across Johns Hopkins.”

How the work lives on at Johns Hopkins

“Now, 20 years later, our Hopkins Community Connection team continues to innovate and expand — working alongside patients, families, clinicians, health system leaders, and community partners.

We are incredibly grateful to Health Leads for setting us on a path that has truly transformed care at Johns Hopkins.”

Venus Ginés, M.A., CHW

President & Founder, Día de la Mujer Latina

A community health worker and three-time breast cancer survivor, Venus has spent her career advancing health literacy, fighting for promotoras de salud, and speaking plainly about the barriers community-based organizations face in getting resourced for work they already do.

Why co-designing with, not for, communities matters

“Health Leads recognizes that Community Health Workers/Promotoras, doulas, navigators, and caregivers are not simply participants in the process; they are essential partners in designing, implementing, and sustaining meaningful change.

Rather than viewing community organizations as advisors on the sidelines, Health Leads created space for organizations like ours to help co-design strategies, share lived experiences, and elevate the voices of those most impacted by health inequities and barriers to care.”

On what the caregiving movement needs now

“Lasting infrastructure is built by investing in the people who serve communities every day.

That kind of leadership, trust, and collaboration is exactly what the CHW/Ps and caregiving movement need to strengthen communities and improve health outcomes for generations to come.”

Learnings & Resources

Core to our mission has been convening community and sharing what we’ve learned from our work — both the successes and the failures — so we can spread what works and move beyond what doesn’t.

So many partners have been integral to bringing these materials to life. Together, these resources reflect the evolution of our work and the lessons we continue to carry forward.

Help Power the Next 30 Years

This anniversary is a celebration of 30 years of innovation, partnership, learning, and community-led change — and an invitation to help shape what comes next.

Our three-year strategic plan builds on our decades of experience investing in the local care workforces and caregivers that are so critical to our health. Through our tested and proven approach to community co-design, our initiatives will focus on the advocacy, financial sustainability, well-being, and integration of this workforce into our systems.

Your gift supports Health Leads as we continue working toward health, well-being, and dignity for every person, in every community.
