Originally designed to support Health Leads’ workshop participants, the Action Plan guides participants through seven design stages that align with the key drivers we have identified as essential to a successful social needs program.
The Action Plan is best used as a “living document” that can be iterated upon as information is shared, resources are obtained and decisions are made. The plan can help facilitate cross-organizational conversation about how to address gaps in care and manage change within your organization.
