Launched by Health Leads in 2017, the Collaborative to Advance Social Health Integration (CASHI) sought to make social health interventions a standard part of primary care across 21 sites in a mix of urban, rural, and suburban settings across the United States. Participating teams set out to develop a business case to demonstrate the value of social health health integration to health system leaders, health plan leaders, and other relevant stakeholders. Produced under the Commonwealth Fund grant, this issue brief provides key insights from the collaborative and practical guidance to support the spread and sustainability of social health interventions in varied institutional and market settings.
