With mounting evidence measuring the impact of social needs on healthcare quality, utilization and cost, interventions that connect patients with the basic resources they need to be healthy have never been more critical.

In the Health Leads Prepare Workshop, our team of experts will guide you through the critical decisions, best practices and tools for building your own tailored social needs intervention. Through weekly webinars, individualized coaching calls and engaging activities in-between sessions, this five-week workshop series will accelerate your organization’s efforts to launch or refine a successful social needs strategy.

Alongside peers from a range of healthcare delivery organizations, participants will take a deep dive into program design, implementation and measurement and gain exclusive access to:

      • A diverse and engaged peer network of stakeholders within the health care and human services sector
      • Unlimited access to Health Leads’ tools, resources, and course materials, including webinar recordings and PowerPoints
      • Curated course discussion and peer-to-peer exchanges via our online Learning Network platform
      • 1:1 coaching from Health Leads experts including tailored feedback and 1-month and 6-month follow ups
