Looking to launch a social needs program, but don’t know where to start? The Action Plan Workbook is designed to guide organizations like yours through key, early design choices to help implement a systematic approach to identifying and addressing address unmet essential needs in the community.

Originally designed to support Health Leads’  workshop participants, the Action Plan guides participants through seven design stages that align with the key drivers we have identified as essential to a successful social needs program.

The Action Plan is best used as a “living document” that can be iterated upon as information is shared, resources are obtained and decisions are made. The plan can help facilitate cross-organizational conversation about how to address gaps in care and manage change within your organization.
