Welcome To The Health Leads Network!
For more than two decades, Health Leads has worked hand-in-hand with health systems to connect patients to the essential resources they need to be healthy. In that time, we’ve seen incredible progress as more and more healthcare and social service organizations take significant steps together to improve health and well-being in their communities.
Today, more than ever before, addressing essential needs is a far more integrated and embraced aspect of care delivery. From urban centers to rural towns and villages, there’s incredible work underway to address the resource gaps that deeply affect individual health and drive broader inequities. And yet, there are so many hurdles to universal adoption and overall sustainability. Opportunities for reimbursement are still few and far between. The work is largely undervalued. Turnover among front-line staff is a continuous concern. And we have a lot of work ahead to elevate the voices of vulnerable populations to drive adoption of appropriate and equitable solutions.
That’s why we’re so excited to bring practitioners together in new ways through the Health Leads Network. Across the country, we see promising practices that provide the structure and support care teams need to succeed — and there’s incredible opportunity for organizations to learn from and grow with each other.
Your work to actively address patients’ essential needs and break down systemic barriers to health makes you a valued member of the Network. In turn, we commit to focus on your biggest challenges — whether it’s the sustainability of your staff, the data needed to understand resource gaps and progress, or practical steps to bring patient and community voices to the forefront. We will work tirelessly to surface the most relevant practices that can contribute to the success of your program.
