One of the most challenging elements of healthcare organizations’ essential needs work is establishing the strong, collaborative relationships needed to connect patients with community-based resources and coordinate these elements of care. But a host of innovative healthcare-community partnerships have already demonstrated a clear path forward for those interested in pursuing more equitable models of care.
Among these innovators is Oregon-based Rogue Community Health, a Health Leads partner and CASHI participant that has established the Community Hub – a regional referral network of healthcare and community service providers that’s supported by a shared, closed-loop screening and resource referral platform.
This case study highlights Rogue’s work alongside community partners to build trusting relationships and structure Hub workflows to support effective collaboration and communication. Combined, the Hub model helps to ensure that community members can access the resources they need, when they need them – improving health outcomes along the way.
